There isn't anything "official" that I have, I do a lot of eye-balling, but the dimensions and score lines for the card are as follows:
Cardstock base: 5 7/8" X 3 1/2" scored at 2 1/2" and 3 1/4"
Lip gloss slits are 1 1/4" long (very important as they will fall through if bigger), you will need 2 of these slits. I just eye-balled it as far as where to start the first one, and how far apart to make them, but the measurements for mine are at 1 1/2" and 2" from the top of the base. Also, important is to cut into the right panel (or back of card) versus into the left panel, as this will cut into the front of your card.
I just eye-balled the ribbon holder slit as well. As long as you have both lined up to match, placement really doesn't matter. Mine are at 1 1/2" from the top. Embellish as desired.
